- Once your request form is submitted, AND APPROVED - A 25% deposit is required at booking time.

- If delivery is over a 15 mile radius, there will be traveling fees added to your balance.

- Balance needs to be paid in full 3 days prior to delivery/pick up date.

- DATES/SERVICE IS NOT RESERVED UNTIL DEPOSIT IS PAID.
- Clients will also be issued a contract/invoice separate from this form.
